Registration Information

Pre-Registration:

For your convenience you may call to pre-register for your appointment. This will reduce check-in time prior to your appointment. To pre-register, please call Hudson Hospital & Clinics at (715) 531-6000, or call toll free (800) 993-2325 and ask for Registration. Hours of operation are 7 a.m. to 4 p.m. Monday through Friday.

Registration:
Patient Registration/Admitting is to the left just inside the main entrance. We ask that you check in at the Information Desk and wait to speak with an admitting clerk, after which you will be directed or escorted to the proper department for your appointment. This helps us efficiently manage patient flow.

During the registration process, you will be asked questions by our admitting personnel. This information is necessary for patient identification, medical record accuracy, and correct and efficient billing. We appreciate your cooperation. All personal information is held in the strictest confidence.

The registrar will collect or update patient demographic information, have the patient sign required forms, scan drivers license, insurance cards and other documents, and collect co-pays. It’s a good idea to arrive 15 minutes prior to the scheduled appointment time to complete this process.

What to Bring for Registration:

    Insurance Card(s): Hudson Hospital & Clinics needs to verify your personal and insurance information each visit to protect your identity. Hudson Physicians Clinic and Hudson Hospital are two separate businesses; you may be asked to show your card twice in one visit if having services at both locations.
    Identification Band: You will be given an identification band . It provides identification to all those who serve you and is for your protection. You may be asked multiple times to verify your biethday and spelling of your name; this is for your safety as a patient.

    Photo Identification Card or Driver’s License
    – to verify patient identification and prevent identity theft.

    Physician's Order:
    if your doctor has given you one (your doctor may fax this order directly to the hospital)

    Co-pay for visit:
    Similar to your visits to your physician’s office, we require your coinsurance, and or co-payment to be paid at the time of service. We accept cash, check, and credit/debit cards.

    Power of Attorney Forms:
    If you have an active POA, please bring a copy, if you know there is not one on file. The POA form will need to be presented in order to be accurately observed. For your convenience, we recommend that you keep a copy of your POA on file at the hospital.

    Legal Guardianship Papers:
    If you have a legal guardian, you must present notarized legal forms for us to have on file in your medical record. Your legal guardian must accompany you for your visit.
    List of Current Medications Unless your physician instructs you otherwise, we ask that you do not bring any medications with you. However, bringing a list of all medications you are currently receiving, and how often you take them may be helpful when seeing your care provider.
